Preschool Screening and Kindergarten
Registration
Preschool screening and Kindergarten
registration will be held on Thursday, March 11th and Friday, March
12th at St. John’s Lutheran Church on Green Street (behind the fire
station). Children aged 3, 4, and 5 are eligible for screening. The
areas of screening include vi-sion, hearing, speech, and development.
Screening is an excellent opportunity for parents and educators to
dis-cuss skills that are necessary for early school success. In order
for children to be considered for our preschool pro-grams, they must be
screened on these dates. Appointments can be made by calling
(815)498-2314 ext 233. Please be prepared with your child’s name, birth
date, desired date and appointment time, and most impor-tantly your
phone number.
Kindergarten
Registration
Kindergarten registration for the
2010-2011 school year will be held at the same time and location.
Children must be 5 on or before Septem-ber 1st, 2010 to be registered
for Kindergarten. Parents need to bring their child’s county birth
certificate and social security number to registration. In accordance
with state law, hospital birth certificates are not accept-able. If you
are new to the district, you will also need to show proof of residency
at this time. This is done by filling out a Certificate of Resi-dence
form and showing at least three of the following:
1. Driver’s license
2. Home ownership title or deed
3. Apartment lease
4. Voter registration
5. Utility bills (water, gas, electric)
6. Automobile insurance
Parents with questions or concerns
about children aged 2 ½ or younger may also call for a
screening appointment.
